29 CFR 1910.134(e)(2)(ii): The medical evaluation did not obtain the information requested by the questionnaire in Sections 1 and 2, Part A of Appendix C of 29 CFR 1910.134: HOSPITAL-WIDE: On or about June 8, 2022 and ongoing, the medical evaluation questionnaire used by the employer to obtain information prior to N-95 respirator fit testing and use did not include all the questions in Part A, Sections 1 and 2 of Appendix C of the Respiratory Protection Standard. The employer requires employees to wear N95 respirators while providing care and when working with to suspected and confirmed positive COVID-19 patients.
