Lewis Memorial Christian Village located at 3400 W Washington in Springfield, IL. On or about June 2, 2022, the employer failed to ensure the COVID-19 log contained at least one form of contact information, the date of the employee's last day at the workplace and the date the employee first had one or more COVID-19 symptoms, if any were experienced.
