29 CFR 1910.147(c)(4)(ii)(B): The employer did not establish specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: (a)Forest Products Distributors, Inc. at 5406 Old Folsom Road, Rapid City, SD 57703: On or about and at times prior to May 12, 2022, the employer did not ensure specific procedural steps for shutting down, isolating, blocking and securing Pellet Mill Machines 1 and 2 effectively removed or controlled hazardous energy in that pneumatic energy was still stored in the machines after written lockout/tagout procedures were performed. This condition exposed employees to caught-in and crush hazards associated with unexpected energizing, startup or release of stored energy.
