29 CFR 1926.501(b)(7)(ii): Each employee at the edge of a well, pit, shaft, or similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers: On April 28, 2022, and times prior thereto, at this construction site employees were installing a sewer line lift station in an excavation. The employer failed to provide fall protection for an employee required to work on top of an approximately 14-foot-tall open lift station.
