29 CFR 1910.119(d)(3)(ii):The employer did not document that equipment complies with recognized and generally accepted good engineering practices. a) Plant; on or about April 25, 2022, the employer failed to determine and document compliance with recognized and generally accepted good engineering practices to protect employees exposed to anhydrous ammonia hazards as specified in ANSI/IIAR 2-1999 Equipment, Design, and Installation of Ammonia Mechanical Refrigeration Systems, section 7.5 Field Leak Testing Requirements, for the testing of the 16 in. diameter suction pipeline after weld repair.
