29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device: This violation occurred on or about April 29, 2022, where employees doing maintenance work in the production Plastic Department on the Chudong Plastic machine T703, Model no. SL-520 Serial no. 0190931. The employer did not ensure employees doing maintenance work on the Chudong T703 machines have tags identify who applied the lock. Employees were exposed to a potential amputation hazard. (a) Maintenance employee did not have his name identified on the tag on the machine. (b) Supervisor applied a lock and not a tag to identify who applied the lock.
