29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a. On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure employees utilized lockout/tagout procedures in that employees grease the forge welder daily and are exposed to pneumatic and electrical hazards and have not been audited. b. On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure employees utilized lockout/tagout procedures in that employees change the dies on the C Frame-Press and are exposed to electrical and hydraulic hazards and have not been audited. c. On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure employees utilized lockout/tagout procedures in that maintenance employees lock out equipment with electrical, pneumatic and hydraulic hazards and have not been audited.
