29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Public Works, Heat Plant Shop #81: The secondary hot water pump 003 was taken apart for service and an incoming water line was not locked out or tagged out. The pressure on the line was approximately 45 psi. Employees potentially exposed to water under pressure. b) Public Works, Heat Plant Shop #81: The hot water boiler 100 and 200 were shutdown for maintenance and each hazardous energy source was not locked out. The supply water and natural gas lines were not locked out or tagged out. Employees potentially exposed to natural gas and water.
