29 CFR 1910.147(d)(2):The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) On or about 3/9/2021, and at times prior, at 110 Plugs Drive, Decatur, AL 35601, on Line 49, employees working to thread/clear jam of sealing substrate on VisionPak employees were exposed to unguarded moving machine parts which could cause contusions, fractures, or disfigurement including amputation because the machine was not turned off or shut down using the procedures established for the machine before accessing the point of operation.
