29 CFR 1910.140(c)(18): The employer did not ensure that personal fall protection systems are inspected before initial use during each workshift for mildew, wear, damage, and other deterioration, and defective components are removed from service. On or about March 8, 2022 and at times prior to, employees exposed to a fall hazard when using personal body harnesses and lanyards which show defects such as, but not limited to, rusty components, frayed nylon, and illegible manufacturer product labels.
