29 CFR 1910.178(l)(4)(ii)(B):Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: At the workplace located at 989 East Commercial Street, Mansfield, Missouri 65704, employees were exposed to struck-by powered industrial truck hazards. A powered industrial truck operator who was involved in an accident that injured another employee was not provided refresher training before returning to work operating powered industrial trucks.
