29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: On or about February 01, 2022, employees were exposed to live electrical parts (2500 KVA) when performing assessment, such as identifying oil leak at Transformer Substation PB46 at W. Silver, Inc. Live electrical parts were not deenergized, safety related work practices were not used to protect against arc flash, arc blast, and electrical shock hazards.
