29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) Monarch Art Plastic Company LLC. DBA Monarch Art Plastics LLC. - Throughout manufacturing plant: The employer did not ensure that authorized employees were trained on all aspects of the energy control program and all specific Lock Out / Tag Out (LO/TO) procedures for machinery including but not limited to: Laminating Presses, Hydraulic Sheer, Die cutter machines, printing presses, punch presses, etc. The machines have multiple energy sources. Employees conducting minor maintenance, cleaning, and unjamming operations were not provided with the Lock out/Tag out procedures, occurred on or about 01/27/2022.
