29 CFR 1910.178(p)(1): A powered industrial truck was found to be in need of repair, defective, or in any way unsafe, and the truck and was not taken out of service until it had been restored to safe operating condition. On or about 1/26/2022, Production Castings Inc. had not ensured a defective powered industrial truck was removed from service until restored to a safe operating condition. An employee was operating a EKKO Forklift - Model: EK15A, S/N: Ek2101B0065. The forklift's brake, reverse and warning lights were missing or broken/inoperable.
