29 CFR 1910.119(f)(1)(ii): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information that addressed operating limits. On or about January 15, 2022, in MDI-1 unit at Rubicon located in Geismar, LA, workers were exposed to chemical and thermal burns when the employer did not implement the operating procedure to safely operate the Dichlorination column (AS-4202) within the acceptable operating limits for pressure.
