29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): a) Oak Pointe, LLC located at 96 New Pace Road, Newcomerstown, OH 43832: On and preceding January 11, 2022, the employer did not ensure that forklift operators were certified to show that they had been trained and evaluated. Several personnel within the facility were designated to operate forklift on a regular basis. No certification was completed to reflect that training and evaluation had been completed on these individuals.
