29 CFR 1910.28(b)(3)(ii): 29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. On or about January 3, 2022, and at times prior thereto, employees were exposed to tripping and/or stepping into: (a) A floor hole between the CCM1 and turn table did not have a cover or guardrail system; (b) A floor hole between the turn table and the edge of the shop floor did not have a cover or guardrail system.
