29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: On or about 1 Dec 2021, at U.S. Postal Service Ponce Atocha location, Ponce, Puerto Rico. An employee engaged in a debris clean-up operation was exposed to 2nd degree electrical burns while removing high vegetation of the surroundings of an electrical power sub-station that was not secured or blocked from releasing energy sources. WSP USA failed to inform the Sunset Contractors that the electrical power station was not blocked nor de-energized. NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.
