29 CFR 1910.252(a)(2)(xiv)(A): The employer did not ensure that the supervisor was responsible for the safe handling of the cutting or welding equipment and the safe use of the cutting or welding process: On or about December 09, 2021, and at times prior thereto, the supervisor did not ensure the safe use and process for cutting or welding equipment and work during the pipeline abandonment work. Employees and subcontractor's employees were exposed to the hazard of fire and explosion.
