Location: 85 Jay Street, Brooklyn, NY. On or about 12/10/2021 a) The employer did not ensure each employee who used a rope descent system was trained in proper rigging and usage of the system. An employee using a boatswain's chair fell when the rigging of the system was not installed properly.
