29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: (a) Location - 168 Albion Street, Wakefield, MA On or about 12/07/21, the employer failed to ensure that a powered industrial truck operator safely handled door materials on double-length pallet and on a day with severe wind gust. Employees working in an aerial lift adjacent to the lift area were struck when the load dislodged during high winds.
