29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that the switch gear was deenergized: At the Hino Motors facility, located in Marion, Arkansas, on or about 11/26/2021, where five employees were exposed to electrical shock hazards while cleaning the switchgear in substation #7 where the infeed lugs were still energized with 13,800 volts.
