29 CFR 1910.67(c)(2)(vii): Outriggers were not used and positioned on pads or a solid surface: (a) On or about October 20, 2021 - SE Corner of Riverchase Parkway/Old Highway 31, Hoover, AL, 2021 CMC 92HD+ Arbor Pro Lift, employees were exposed to fall and struck-by hazards when two of the outriggers were placed on pads on soil at the crest of a slope at the edge of a ditch/bowl/lower level while extending the boom to is maximum length while trying to reach a tree limb for removal.
