29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Water System Drilling, Inc. at Box Elder City Well #10 site, Box Elder, SD 57719: On or about and at times prior to October 16, 2021, the employer failed to ensure employees were utilizing lockout/tagout and de-energizing the MD Cowan Super Single drilling rig while performing maintenance on the top drive. This condition exposed employees to struck-by and caught-in hazards.
