29 CFR 1917.50(c)(5)(ii): Special stevedoring gear provided by the employer having SWL of 5 short tons or less was not inspected and tested as a unit before initial use (according to 29 CFR 1917.50(d) and (e) or by a designated person): a) On or about September 25, 2021 at the barge unloading area employees were exposed to struck-by hazards while working around an "I" beam crane attachment used for lifting hold covers and the employer did not ensure special stevedoring gear provided by the employer having SWL of 5 short tons or less was not inspected and tested as a unit before initial use.
