29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about October 20, 2021, the employer, Bricker Wilson Construction Services, LLC, did not ensure each employee was trained in the recognition and avoidance of unsafe working conditions when required to install a water line in a vertical-walled trench of approximately 8 feet in depth. Employees had not been trained in the recognition of hazards when performing trench work. In addition, employees had not been trained on the correct installation of hydraulic shoring.
