29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Cauttrell Enterprises, Inc. located at 7618 N Broadway, St. Louis, MO 63147: On or about October 15, 2021, the employer had not developed, documented and utilized proper procedures for the control of potentially hazardous energy when employees changed the abrasive wheel used to cut steel beams.
