29 CFR 1926.761(b): Fall hazard training. The employer shall train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program. a) Henry Gurtzweiler, Inc. on or about October 14, 2021 at the address of 25925 Commerce Dr. Defiance, OH: The employer did not ensure employees performing steel erection activities atop a commercial structure were adequately protected from falls. Employees installing insulation and roof sheeting atop the structure were exposed to falls of approximately 34 ft. to the ground below. Henry Gurtzweiler, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.760(a)(1), which was contained in OSHA inspection number 1463900, citation number 1, item number 1a and was affirmed as a final order on May 28, 2020, with respect to a workplace located at 26997 N.
