29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: On 10/4/2021, employees of HR Vasquez Construction LLC were performing roofing work at 1006 Regency Manor Drive, Ballwin, Missouri. An extension ladder was placed on the West side of the house and used to access the upper landing surface of the roof. The side rails of the ladder did not extended 3 feet above the upper surface.
