29 CFR 1926.1053(b)(1): Where portable ladders are used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Advanced Contractor Corp. does not ensure that ladders are extended at least 3 feet (.9 m) above upper landing surfaces being accessed. This violation was most recently observed, on or about 10/4/2021 at 45 Wellesly Way, Evesham NJ, instance (a): Employees were exposed to fall hazards, up to and in excess of 15 feet 7 inches to the ground surface below, when accessing a residential structure's pitched roof using a portable aluminum extension ladder whose side rails did not extend at least three feet above the surface being accessed.
