29 CFR 1910.147(c)(5)(ii)(C)(1):Lockout devices were not substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools: Cup Robot C & D area: On or about September 27, 2021, and sometime thereafter, lockout devices including non-locking carabiners, were used to prevent employee access to energized machines such as the C&D conveyors and the palletizing robot (model FANUC Robot M-410 IB160) and were not substantial enough to minimize the likelihood of early or accidental removal.
