29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about 9/11/2021 - Ram Department, lockout procedures had not been developed, documented, and utilized for employees using a tool to remove refractory material from the main conveyor belt during product change exposing employees to caught-in hazards. (b) On or about 09/20/2021 - Facility, lockout procedures had not been developed, documented, and utilized for employees performing servicing and maintenance for equipment such as, but not limited to, the Blue Mixer, Muller, Chopper, Small Outfeed Conveyor, and Packaging Stand exposing employees to caught-in and amputation hazards.
