29 CFR 1910.134(m)(2)(i): The employer did not establish a record of the qualitative and quantitative fit tests administered to an employee which included the information required by 29 CFR 1910.134(m)(2)(i)(A) through (m)(2)(i)(E): (a) Temple University Hospital, Inc. located at 3401 North Broad Street, Philadelphia, PA: On or about September 13, 2021, the employer did not ensure that fit test records included all of the required information such as but not limited to the type of the fit test performed, model, style, and the pass/fail results. The employer required employees to wear N95 filtering facepiece respirators to protect employees against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 patients.
