29 CFR 1910.28(b)(3)(iv):29 CFR 1910.28(b)(3)(iv): The employer did not provide that each employee be protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used. A) At the number 1 Baghouse, the number 1 Cooler, and west exterior of cooler building, on or about September 7, 2021, and or times prior to, the employer exposed employees to fall hazards, in that a self closing gate to access work platforms was not provided for employees utilizing fixed ladders.
