29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: (a) Jobsite located at 3515 Midvale Avenue Philadelphia, PA, 19129: The employer did not ensure that employees working in an excavation use a self-supporting ladder as designed. Employees were exposed to fall hazards while using a portable A-frame ladder to access and egress the excavation in a folded position and not opened according to the manufacturer's specifications. Condition was observed on or about August 27, 2021.
