29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Colorado Roofing Products, LLC at 3590 Himalaya Road in Aurora, CO: On or before August 25, 2021, the employer's lockout/tagout program was not effectively implemented to ensure employee safety during assigned servicing and maintenance tasks. The employer did not ensure that machinery was properly isolated and rendered inoperative before any employee performed any servicing or maintenance on the equipment. Operations employees were not required to lock and tag out equipment when performing servicing and maintenance tasks that fell under the lockout/tagout standard, nor did they receive authorized employee training. This condition exposed employees to amputation hazards and hazardous energy.
