29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: The employer is failing to protect employees from caught in hazards due to unguarded rotating shafts. This was most recently observed on August 17, 2021 at 8211 F St, Omaha, Nebraska where an unprotected gear box drive shaft protruded 3 inches outside the case, exposing employees to injury.
