29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. Oldcastle APG West Inc. dba US Mix Co. at 112 S Santa Fe Dr., Denver, 80223: On and before August 16, 2021, employees were not protected from hazards created by rotating parts. Employees operated and maintained the conveyor belt and gravity roller conveyor leading to the Robot palletizer, without guards to prevent entanglement in rotating parts. An employee sustained a finger amputation after his hand was caught in the conveyor rollers while attempting to remove a cement bag. This condition exposed employees to amputation hazards.
