29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: A) 879-899 Main St Sayreville, NJ 08872 Employees did not affix lockout or tagout devices to isolate hazardous energy sources to the Trim Press 5.0LRTP Serial 1200 Year 13 that included, but was not limited to the main power disconnect, platen lock pin, air lockout point, gearbox lock and treadle lockout while employees performed servicing and maintenance activities. The activity that was being performed at the time of the accident was to clear a materials jam. Hazard occurred on or about 08/13/21
