29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) State Metal Industries, H Furnace: On or about 12/9/21, furnace operators that were directed to enter the H furnace to clean were not supplied a personal lockout device prior to cleaning. H furnace was locked out by maintenance employees prior to entry of the furnace. NOTE: IN ADDITION TO ABATEMENT CERTIFIACATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENATIONS FOR THIS ITEM. FAILUTE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29CFR 1903.19
