29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. a-34 Maplewood Boulevard, Suffern NY: Employees engaged in residential roofing activities using a portable aluminum extension ladder to gain access to the 5 on 12 pitched roof were exposed to fall hazards of approximately 18 ft to the concrete below as the ladder side rails were not extended 3ft above the upper landing surface.
