29 CFR 1910.119(h)(3)(ii):The contract employer did not assure that each contract employee was instructed in the known potential flammable and toxic release hazards related to his/her job and the process: On or about July 27, 2021 in the Acetic Acid Unit of the LyondellBasell Chemical manufacturing facility in La Porte, TX, workers were exposed to process materials, toxic release, and flammable hazards when pipefitters were not instructed in known hazards associated with work being performed on HV-117. a) Pipefitter foreman was not instructed on known hazards associated with work being performed while working on HV-117. b) Pipefitter 1 was not instructed on known hazards associated with work being performed while working on HV-117. c) Pipefitter 2 was not instructed on known hazards associated with work being performed while working on HV-117.
