29 CFR 1910.146(c)(5): The employer did not use the alternate procedures specified in paragraph (c)(5)(ii) of this section for entering a permit space under the conditions set forth in paragraph (c)(5)(i) of this section: On or about 7/15/2021 and at times prior, Casting Machine #818, employees cleaning core material that had collected under the machine were exposed to machine hazards associated moving machine parts and in-going nip points when alternate procedures were not utilized.
