29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. Beckett Air, Inc. - North Ridgeville, OH: On or about July 1, 2021, the employer did not ensure the point of operation on Amada press brake No. 00523 (model 3512LD) was adequately guarded to protect employees from crushing and amputation injury. b. Beckett Air, Inc. - North Ridgeville, OH: On or about July 8, 2021, employees that operated the Roper Whitney, Inc. roll bender machine (model 3418-H) were exposed to amputation injury. The employer did not ensure the machine's point of operation was guarded.
