29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: The employer is failing to protect employees from struck-by and cave-in hazards while in excavations. This was most recently documented on or about July 02, 2021 at Underground Specialists, Inc. jobsite located at 2015 Andover Dr. Garden City, Kansas. An employee entered an excavation that measured approximately 5 feet in depth to assist a backhoe operator locate and expose a water line. The employee descended into the excavation by climbing.
