29 CFR 1926.754(e)(2)(ii): The employer did not deck over roof and floor holes and openings. The employer is failing to protect employees from fall hazards associated with the uncovered hole openings. This was most recently documented on or about June 29, 2021 at jobsite located at 2590 CR L, Ashland Kansas, where the employer has employees perform construction activities on a new flake room without decking over a floor hole opening exposing employees to a fall hazard approximately 21 feet above the concrete work surface below.
