29 CFR 1910.134(e)(1):The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: (Construction Reference 1926.103) Employees were exposed to particulates otherwise not regulated (total dust) concentrations greater than the OSHA permissible exposure limit (PEL) of fifteen milligrams per cubic meter (15 mg/m3) of air averaged over an eight-hour period. Employees were performing spray insulation activities were exposed to particulates otherwise not regulated (total dust) hazards of 78.3, 40.6 and 61.2 mg/m3 and while wearing N-95 respirators and the employer did not provide a medical evaluation to determine the employee's ability to use a respirator in the following job-sites: 1. East Sweet Gum Dr. Wichita, KS 2. 26th St N Delano, KS
