29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) 447 Production, Rack Shaping, Koenig: Employees were not provided locks or other devices so they could lockout the machine and it's components while employees cleaned areas of the machine that contacts dough or may accumulate food residues, including but not limited to rotating forming drums, finger belts, various rollers, and conveyor belts. This exposed employees to the hazard of being caught by an in running nip point on or about June 11, 2021.
