Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about June 3, 2021 the employer failed to ensure that effective machine guarding was in place on the Serac 2 milk stacking and palletizing unit. Employees were exposed to the moving machine parts including the up and down movement of the pallet table and accompanying shafts/ supports. Employees are able to reach into the hazardous area when standing on the exit portion of the equipment next to the exit conveyor. The barrier guard which was present on the exit side of the equipment at one point was no longer installed. b) On or about August 12, 2021 the employer failed to ensure that effective machine guarding was in place on the FS Robot (milk container stacking/palletizing robot cell). The rear barrier guarding did not extend to the wall to prevent employees from walking into the robot cell during operation. The interlocked door in the area contained an interlock device which did not function. The employer placed a chain and lock on the door to prevent it's use encouraging employees to walk around it. c) On or about August 12, 2021 the employer failed to ensure that effective machine guarding was in place on the cardboard former unit. Employees were observed operating the equipment where the interlocked door was missing barrier guarding to prevent employees from reaching into the moving machine parts of the equipment. d) On or about August 12, 2021 the employer failed to ensure that effective machine guarding was in place on the palletizing conveyor line. Employees were observed making adjustments to the pallet of milk containers while in movement down the powered conveyor line. Employees were exposed to the pinch point created between the moving pallet and vertical bollards adjacent to the line. e) On or about August 12, 2021 the employer failed to ensure that effective machine guarding was in place on the Serac 2 milk stacking and palletizing unit. Employees were exposed to the moving machine parts including the up and down movement of the pallet table and accompanying shafts/ supports. Employees are able to reach into the hazardous area when standing on the entry portion of the equipment next to the exit conveyor. Employees walk around the left side of the equipment to the stacking area where a barrier guard is not present to retrieve fallen containers. Superior Dairy, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.212(a)(1) which was contained in OSHA inspection number 1324154 , citation number 1, item number 1, and was affirmed as a final order November 9, 2018 with respect to a workplace located at 4719 Navarre Road SW, Canton, OH 44706
