29 CFR 1910.25(b)(4): The employer did not ensure that stairway landings and platforms are at least the width of the stair and at least 30 inches (76 cm) in depth, as measured in the direction of travel. Flagpole Base, Parking Lot B: Two employees were untangling flag lines from the stairway upper landing/platform located above the base of the flagpole foundation that was 15 inches deep. One employee lost his balance and fell 37 inches into the concrete sidewalk and sustained a fatal head injury.
